Sally Fay Cook-Clevenger

Sally Fay Cook-Clevenger, age 63, of Reading, OH and formerly of West Alexandria, OH passed away on Wednesday, May 12, 2021 at Bethesda North Hospital in Cincinnati, OH.  She was born March 11, 1958 in Dayton, OH to the late Earl Thomas and Geneva Cook.  She is survived by her daughter Sarah Clevenger of Reading, OH; son Allen Clevenger of Reading, OH; son Jimmy Clevenger of Reading, OH; 6 grandchildren; brother Thomas (Sheila) Cook Verona, KY; brother Michael Cook of West Alexandria, OH; and numerous nieces and nephews. Visitation will be held on Wednesday, May 19, 2021 from 11:00 am to 1:00 pm at the Lindloff Zimmerman Funeral Home, 67 West Dayton Street, West Alexandria.  A graveside service will folllow at 1:30 pm at Fairview Cemetery in West Alexandria, OH.  Memorial contributions may be sent to the funeral home to assist with final expenses.  Online condolence and other remembrances may be sent by visiting www.lindloffzimmerman.com.
